INSURANCE RATING

PREFERRED OR STANDARD: The medical underwriting of life insurance policy is critical because the condition determines how an individual will be rated and the cost he must pay for the policy. If a client can answer no to all the questions below, there is a good chance they can qualify for the better rate of preferred.

1. Has the insured used tobacco products within the last 3 years.

2. Has there been death by coronary artery disease before age 65 among family members

3. Is the insured's weight greater than the maximum listed for the corresponding height

4. Has the insured ever been treated for elevated blood pressure

5. Has the insured ever been treated for elevated cholesterol

6. Does the insured have knowledge of current cholesterol levels that exceed published guidelines

7. Is the insured employed in a hazardous occupation

8. Within the past two years, has the insured been convicted of any DWI's or more than 3 moving violations

9. Within the last two years, has the insured participated in a hazardous avocation or aviation activities

10. Is the insured currently in the active military or reserves

11. Has the insured been diagnosed or had a history of significant medical impairment such as diabetes, cancer, cardiovascular, respiratory, liver or kidney disease.

Cholesterol Guidelines: Age 18- 45 Not exceeding 200 mg% , 46- 65 220 mg% , 66- 75 240 mg%
